Mammogram, Pap Smear and Prostate Exam, PSA must be current to participate in Hormone Replacement

Laboratory Testing - Required to Get Started- $199- Fee Includes follow up laboratory monitoring for 1- calendar year as 

determined by the Nurse Practitioner.

Saliva Testing to assess levels is also available upon request (Saliva Testing Pricing $399 to $499)

*Not all clients are suitable candidates for Hormone Therapy. Participation in a consult 

 does not guarantee hormone therapy will be prescribed. Labs must support need, and 

 patient may not have contraindications to treatment.

Testosterone Replacement Membership- $154 per month- Recurring ($1848 per year)

Treatment  includes routine laboratory testing completed at Revive Wellness & Health Clinic 

Routine Office Visit with Nurse Practitioner and monitoring of hormone levels ( After 6 weeks of treatment , 3 months and then every 6 months while on treatment, or more frequent as determined by provider ) 

Monthly B-12 Injection 

Bio-Identical Hormone Therapy  (creams, sublingual spray, tablet, or troche as determined by provider) up to 2  products as determined by provider. 

Hormone Replacement Membership- $99 per month- Recurring ($1188  per year)

Treatment includes routine laboratory testing completed ar Revive Wellness & Health Clinic

Routine Office Visit with Nurse Practitioner and  monitoring of hormone levels ( AFter 6 weeks, 3 months, every 6 months while on treatment ) is required

Monthly B-12 Injection

Provide patient a  prescription for Hormone Replacement Therapy (medication  is sent to local compound pharmacy or pharmacy of your choice)

*We do accept FSA, HSA 

*Hormone Pellets and Pellet Procedure will be an additional cost, and is not part of the HRT Package.

* Participation in the HR membership plan does not guarantee  hormone therapy will be prescribed. Labs along with history and physical exam

   must support need for treatment. HRT will not be prescribed when need is not supported by laboratory values and patient symptoms.
